Dear UofT Part-time Undergraduate Students,

APUS (Association of Part-time Undergraduate Students) is a 40+ year old student union that represents part-time (p/t) undergraduates at the University of Toronto, based at Woodsworth College. APUS advocates, organizes and offers services that are geared to benefit and enrich the lives of p/t students. We have a proven track record in a variety of areas and once again, need to hear your voices so we can offer even better services for you.

WHO IS A PART TIME STUDENT?
You are considered a part-time undergraduate student and a member of APUS if:
- you are taking 2.5 credits or less during the September through April academic year or;
- you are taking 1 credit or less during the whole Summer session.

PURPOSE OF THIS HEALTH & DENTAL PLAN SURVEY – TO IMPROVE BENEFITS!
This survey is designed to obtain feedback from ALL our part-time members about the type of improvements that would benefit the majority regarding APUS’ Health & Dental Plans. There are ONLY two options to consider this year and selecting ONE of them will direct us on which improvements you think are best for the APUS’ Health & Dental Plans in the upcoming 2011-2012 academic year.

We are limited in the number of benefits we can add per benefit year but this will make a big difference so please assist us by taking the time to answer the question thoughtfully.

Below are the two options we are considering based on tracking student inquires and analysis of the claims part-time student have made in the last few years.

Please choose ONE of the following options:

Option A:
Health Plan: Increase the per paramedical maximum from $15 to $25 per visit
Dental Plan: Increase dental maximum from $500 to $600 per benefit year & Increase dental coinsurance from 60% to 65%

Option B:
Health Plan: Add pay direct
Dental Plan: Add coverage for root canal at 60% coinsurance

This survey has expired on July 31, 2011.
